If you choose to homeschool according to Option 2 in West Virginia, fill out this report and mail it “certified mail/return receipt requested” to the superintendent of your school district on or before the date beginning home instruction and when moving to a new county. You will also need to provide evidence the person providing instruction has the necessary qualifications.
