There are two options for homeschooling in the state of Louisiana. If you've chosen to homeschool as a home-based private school, you may use these forms as instructed below.

If you wish to report your attendance as a private school, you can do so online through the Louisiana Department of Education website here or fill out this notice and mail it Certified Mail/Return Receipt Requested to:

Mail it no later than 30 days after starting your school year. Your report of attendance is to be done annually.
If you report as a private school and your child has been attending public school, also fill out the “Notification of Enrollment in a Private School” form and mail it to the principal of the public school your child was attending.
